SUPPORT
CO-ORDINATION
Our capable team has the experience and know-how to help you get the most out of your NDIS plan. We will make sure all the supports in your plan compliment each other to help you achieve your personal goals. Firstly, we listen to you to understand your situation. Then, we collaborate with you, service providers, community and government partners to build a program of care with your individual needs in mind.
SPECIALIST SUPPORT
CO-ORDINATION
For when your situation includes additional high or complex needs, we help you manage the challenges you're facing in your environment. Our aim is to reduce complexity in your systems of support, help you negotiate solutions (including in emergency and time-limited situations) and empower you to build your capacity and skills.
PSYCHO-SOCIAL RECOVERY COACHING
Psycho-social recovery coaching falls within the Support Co-ordination NDIS funding stream and is largely about capacity-building for people with mental health conditions. With this service, we help people who may be leaving hospital or living in the community with mental health issues and coach them towards recovery. We go beyond to ensure you have the support you need, when you need it. We help you to navigate your recovery and coach you in decision-making and other areas of daily life. We're in the business of building your confidence to assist in your journey to reconnect with society.
